1.  Print out and complete the form

 

2.  Write a 3—5 page essay explaining why you desire to become a nurse and       what areas of nursing you wish to practice.  Also explain what adversity           you have had to overcome to obtain your dream of becoming a nurse.  The      essay should be double spaced, 12pt font, MLA format.  Pay close                attention to spelling and grammar.

 

3.  Obtain at least one letter of reference from a faculty member

 

4.  Mail completed application, essay, reference letter and college transcripts to:  Michele Henderson Scholarship

     PO Box 2110

     Marshall, TX 75671

 

Applications must be received on or before April 30, 2010
